GETTING AN ASSESSMENT

Our process begins with an assessment that allows our trained professionals to gather some information and determine what’s going on. When you call Peachford Hospital, our admissions team will ask you a few questions in order to schedule a face-to-face appointment. During an in-person assessment, a mental health professional will continue to gather information This allows our team to make a recommendation about the best course of treatment for your unique situation.

You can save time by downloading our Assessment Packet, to get a head start on filling out some of the necessary paperwork.

If you’re a behavioral health professional looking to get some help for your client, you can also fill out our Professional Referrals Form to help expedite the admissions process.
